Note that you need Cydia to create this look.

I used:

"White SB Font"
"Simple iPad Theme"
"No Spotlight Icon"
"No Page Dots 3.0"
"UK O2 Logo. Theme"
"No Docked Icon Labels"
"Shrink App" at 66% original size.
